Stage 1 – Medical & Provisional Licence

Before you apply for your provisional licence you will need to take a medical

We can arrange this for you at a local medical centre

We will send you the necessary paperwork to be completed, and advise you with regards to arranging your medical.  Once this is done; all you have to do is send off your completed paperwork to the DVLA.

The first stage in gaining your LGV/HGV driver licence is obtaining your provisional entitlement.

The D2 form is an application for provisional entitlement for LGV/PCV, this has to be completed by the applicant.

The D4 form is the Medical Report, this has to be completed in the presence of a Doctor and signed by both of you.

Both the D2 & D4 need to be sent to the DVLA at Swansea.

There is no charge to have the Vocational entitlement added to your current Driving Licence, however there is a charge for the medical.
