Stage 1 – Medical & Provisional Licence

Step 1: Apply for Your Provisional LGV/HGV Licence

Before you can apply for your provisional LGV (Large Goods Vehicle) or HGV (Heavy Goods Vehicle) licence, you’ll need to undergo a medical examination.

We can help arrange your medical appointment at a local centre.
We’ll also send you the necessary paperwork and guide you through the entire process.

What You’ll Need:

D2 Form – This is the application for provisional LGV/PCV entitlement and must be completed by you.
D4 Form – This is the medical report, which must be filled out and signed by a doctor in your presence.

Once both forms are completed, they must be sent to the DVLA in Swansea.

There is no charge to have the vocational entitlement added to your driving licence. However, please note that the medical examination does carry a fee.
